
Your designated beneficiary will receive a lump-sum benefit upon your death. It would help if you were covered by the Health & Welfare Plan for your designated beneficiary to receive this benefit.
The Plan also provides an additional benefit to active employees for accidental death or dismemberment. You may also be eligible to receive up to half of your life insurance benefits if you are terminally ill.
The amount of the benefit that your beneficiary is eligible to receive is based on your work status and the other coverage you have, as shown in the chart below.

Supplemental Life Insurance Exclusion
The supplemental life benefit is not payable if you commit suicide within two years of obtaining the coverage.
